PAYMENTS & INSURANCE

I am a MnFIRE provider and partner with the MnFIRE Peer Support program, managed by PRO Network. You may obtain 5 free sessions for any firefighter in the state through MNFIRE.

I currently accept private pay for services and some in-network insurances such as Aetna, Carelon Behavioral Health, Cigna, Horizon Blue Cross Blue Shield and Quest Behavioral Health. If you wish to use private pay, I will provide you with a receipt to use for reimbursement for some insurance carriers.

Payment is due at the time of service. I accept cash, check, credit or debit, or HSA.

CANCELLATION POLICY

Please be mindful when reserving time for your appointment and remember that payments are due at the time of service unless other arrangements were made prior. If you need to cancel or reschedule your appointment, please provide a minimum of 24 hours advanced notice. If you're unable to let me know by email, phone or by cancelling/updating your appointment on this site, you may be charge a $100 late cancellation fee.
